同治皇帝病重，青春李莲英（姜文 饰）力荐慈禧（刘晓庆 饰）二次垂帘听政，青春不久同治身亡，慈禧立载湉为帝，继续把持朝政。1882年，李莲英陪同醇亲王巡视北洋水师，返京后却遭都察院纠弹，被革职处理，在慈禧的主张下，李莲英官复后宫大总管原职。李莲英为使侄女有个好归宿，将其带入宫中， 岂料只落得奴婢下场，他心中充满愧疚。光绪日渐成长，不甘受慈禧摆布，试图从慈禧手中夺权却被强力镇压，李莲英畏惧慈禧淫威，奉命将珍妃处死。不久八国联军入侵，清军连战连败，李莲英随慈禧微服出逃，途中在农户家中落脚，农妇的一席话，令李莲英与慈禧若有所悟……
